Meeting patient needs in the final year of life

CBEH Director Professor Stephen Birch and colleagues have published new research that investigates the needs of patients and the services they receive in the final year of life. Their scoping review of the literature on patient care transitions in the last year of life shows that the changes in patient needs during the period leading to death are often not reflected in the settings in which people receive care. Instead, the care setting is often based on available capacity in a care setting as opposed to the suitability of the setting for meeting needs. Barriers to a more responsive system of care setting transitions include funding models and planning mechanisms that are not based on patient needs.
